What forms can the Disability Advice Project (DAP) help you with?

The Disability Advice Project can assist you with all forms concerned with disability benefits. These include:

Disability Living Allowance (Adults 16 – 65th birthday)
Disability Living Allowance (Children under 16 years old)
Attendance Allowance (Over 65 years)
Carers Allowance
Employment Support Allowance (ESA)
All renewal claim forms

Some forms can be quite daunting and complex. DAP have trained, experienced people to assist you complete the forms. On rare occasions other types of forms require further expert help, but we can advise you on this matter.

What you need to do if you need help to complete a form

Contact the Disability Advice Project office; due to the detailed information that is required to complete such forms, we run an appointment system that ensures you receive individual attention.
